Pulpitis is inflammation of the pulp, or the bundle of nerves and blood vessels located in the soft tissue of the tooth. In children, decay quickly turns into pulpitis, and pulpitis itself, if left untreated, can rapidly progress to inflammation of the tissues around the tooth and end in its loss. It's all down to the features of the anatomy of baby teeth: a smaller crown, minimal thickness of enamel and dentine, a lower degree of mineralisation, a larger volume of pulp and shorter dentinal tubules.
Characteristic symptoms:
- Pain when chewing or eating spicy, sweet, sour, hot or cold food
- Sometimes pain at rest, worsening at night
- A rise in temperature, general malaise, swollen lymph nodes
- Swelling of the gum or cheek in the area of the affected tooth
- Discharge from the tooth or from under the gum, redness, tenderness of the gum
- Most often it is a complication of decay that wasn't treated in time
- As a result of injury to baby teeth; mainly the front group of teeth
- A loss of seal on a filling in a previously treated tooth (pathogens get into the tooth)
Acute pulpitis
It can be caused by injury to the tooth. It is accompanied by:
- sharp pain (radiating to the head and ears, appearing for no reason or when biting down)
- tenderness when breathing in air (occurs after injuries)
- poor sleep, restlessness, tearfulness
- bouts of night-time pain (in children aged 5 and older)
Chronic pulpitis
This is the result of decay that wasn't treated in time, which arose from insufficient hygiene and the action of cavity-causing bacteria on the enamel. In children under 5 pulpitis develops faster than in adults, since their enamel is thinner and the pulp chamber larger. The distance between the hard tissue and the bundle of nerves and blood vessels is small. So the infection reaches the inside of the tooth in a short time.
Chronic pulpitis often runs without symptoms and is only discovered at the dentist's appointment. The child recalls that the tooth used to hurt but then stopped. Yet the inflammation remained. In advanced cases it spreads to the root canals and provokes periodontitis or periostitis.
Before treatment, comprehensive diagnostics are always carried out. The doctor notes the complaints, examines the mouth and takes an X-ray of the affected tooth. Diagnostics for children in Moscow, at Inwhite Medical Kids, may also include computed tomography. If the diagnosis of “pulpitis” is confirmed, the treatment stage begins directly. The therapy is chosen depending on how far the process has developed. In any case it is completely painless and carried out using safe anaesthesia.
Treatment mostly takes place in a single visit to the children's clinic. In this case only the crown part of the pulp (reversible pulpitis) or the nerves from the canals (irreversible pulpitis) are removed. In the first case the affected part of the nerve is removed and the healthy part is covered with a special biocompatible material. The agent hardens and becomes a reliable protection from external disease-causing bacteria. Then the tooth itself is restored. Treatment can also be done in one visit if the inflammation hasn't gone beyond the root canals — then only they are treated. The nerves are removed from the canals, they are washed and dried, and then a biocompatible material is placed into each canal. The tooth is then restored to be sealed and anatomically correct.
Sometimes pulpitis in children is treated in two visits. This usually happens with a borderline state between pulpitis and periodontitis. A calcium-containing agent is left in the canals to destroy the remaining bacteria. The tooth is closed with a strong temporary filling. In about two weeks the canals are finally filled and the tooth restored. Dentistry has methods of treating pulpitis that were long ago recognised as dangerous, outdated and ineffective. They include: treatment using arsenic paste, using pulpotec and leaving the tooth open, and the resorcinol-formalin method. - What complications can pulp inflammation have?
If treatment isn't carried out in time, acute pain arises that prevents the child from living their normal life. Premature loss of the tooth and the development of the most serious conditions are also possible — periodontitis, a purulent abscess, cysts, osteomyelitis (inflammation of the bone) and periostitis (inflammation of the periosteum). With delayed treatment the infection goes beyond the baby tooth, where it can harm the bud of the permanent one. So treating pulpitis in temporary teeth is essential. - What does prevention of pulpitis look like?
Following the rules of home hygiene, along with regular preventive check-ups at the dentist, makes it possible to reduce the risk of decay and to detect dental problems at the earliest stage of development. If decay has already been identified or you've noticed its symptoms yourself (darkening of the enamel, the child complaining of tooth sensitivity or pain), the best prevention of pulpitis in baby and permanent teeth is timely treatment of the enamel damage. - Does acute pulpitis always develop?
Quite often the disease process runs without symptoms until it reaches the acute stage. And only then does the child get throbbing toothache. In the evening or at night the pain worsens from cold or hot. There is often pain when the jaws are clenched or food gets on the tooth. The child may develop swelling of the soft tissue, a rise in body temperature and swollen lymph nodes.
